Computational modelling and simulation are powerful tools for understanding complex fluid flows and their interactions with engineered devices. However, translating these capabilities into clinical applications requires models that address relevant clinical questions while accounting for available patient data and the constraints of existing clinical workflows. This raises important engineering considerations around model fidelity, assumptions, boundary conditions, and fitness for purpose. In this talk, I will explore these challenges through patient-specific modelling of transcatheter aortic valve implantation (TAVI), a minimally invasive procedure to replace a diseased heart valve. Drawing on examples of valve deployment and repeat interventions, I will discuss how clinical needs and available data inform modelling decisions, and how fluid mechanics can help us understand the consequences of device-patient interactions.
